I was having the same issue with a smaller quad i got for the kids - the chain would fall off when we went for a bumpy ride... I found that there was a nut loose on the back frame that caused it to move when in rough terrain...might not be your issue but worth making sure the frame bolts are tight...

How do the sprockets look? When was the last time you replaced them?

the factory chains stretch super easily. it'd be best to replace it with a good quality chain.

yep, then the tensioners, if applicable, would do their job great!

We had problems with chain jumping and popping off and we discovered it was a motor mount bolt had shaken loose and fallen out, letting the engine and its sprocket tilt just enough to throw off the chain alignment. We went through two chains until we noticed the missing bolt. Replaced the bolt and now it runs perfectly smooth and stays engaged all the time.
