Chain problems on 85 Honda trx 125

When I put the chain on it falls off again can anybody tell me why and how to fix it

Could be a few things. Chain could be stretched and needs replaced, sprockets could be worn or missing teeth. Sprockets could also be out of line.

Is it a new chain or new sprockets?
