Default grindin roder?

i was ridin my quad today befor i go to ny tomorrow, and i herd this grinding noise so i checked it out and one side of my roder is smooth anymore, actually its pretty scracthed, i replace the pads about 2 months ago but now the roder is getting ruined. do you think its because i brake to hard or something, or is it something else? and when i pull the handle it feels not as tight as it should be, do i need new pads?

what are you riding in? if you're in sand, and get it between your rotors and pads, it'll do some serious grinding on both.
