Default Swing arm bushings????

My chain keeps popping off my Trailblazer and I can't figure out why. I was wonderin cause my swingarm can move side to side like quarter of an inch could that be the problem?

Thought it was from the motor and tranny mounts at first checked and replaced a few wasn't the problem, it has a new rear sprocket and gear on the tranny neither are bent.
Not the chain guard either brand new.

do you have any missing teeth on your sprockets. is your chain the proper tension. next time the chain is off hold it horizontaly in the air. and if the end opposite what you are holding is more than 6 inches lower i say its time to replace your chain.
