Default Axle Nuts

This weekend I went riding and to get there I have to ride on the pavement. I got to the hillclimbs and I was climbing and my chain poped off, I went around to look and the axle nuts had came loose and the jam nut was all the way off the threads. I tightened them as much as me and my dad could and kept on riding. Never had this problem before and never used any locktight. Has anyone else had this problem, when I got back I tighted them down and put some finger nail polish (quick fix like locktight) on the threads and tightened them. If anyone else has has this problem how did you stop it, and by the way the threads are fine and not stripped or anything.
