A loud exhaust and a slow moving trike that you walk past in high gear isn't an exhaust problem. If you said the exhaust sounded quiet and restrained....maybe, but it sounds to me like your buddies clutch has headed south.
If you can run as fast as the trike runs in high gear, a clutch adjustment isn't going to fix it.
Pull the right side cover from the engine and check the friction plates of the manual clutch assembly. If that's OK....go to your Honda dealer and buy a spanner nut wrench [$15.00] and pull the centrifugal clutch assembly and inspect it. The shoes should have grooves and the hub should not. Both are likely shot if you guys have been running this for a while in this condition.

It does have a manual clutch. I am not referring that it has a "manual operated" clutch.
It has a clutch just like a 200X for example, only that it is engaged by the foot shifter rather than a lever on the handlebars. It's automatically operated, but the part itself is still known as a manual clutch. Most guys refer to them on auto quads as being a "clutch pack"........so maybe that will help you figure out what I am talking about.
