Matt,
If there is such a generic form I've never heard of it. Some manufacturers & suppliers have their own applications but they would only be applicable to themselves.
Possibly the person you talked to was referring to a resume, a personal history of yourself.
Your resume should contain:
1) your full name, age, mailing address & tel no.
2) history of your riding experience
3) detailed list of your racing wins and/or
standings, if any
4) short statement of your goals in racing
5) one or two photos of yourself & quad in
full riding gear & as clean & well-
groomed as possible
As a matter of ethics, never seek or accept sponsorship from two or more companies offering the same service, ie., 2 restaurants. 2 service stations, etc.
Keep in ming that when a firm does sponsor you that you will be representing them and your actions, language, and general attitude will reflect on the sponsor.
Another suggestion, Matt.Save your completed resume as a document in your computer so it can be up-dated & printed again. You can send or give it to prospects with a cover letter, giving one or two reasons why you'd like them as a sponsor (their quality, service, reputation, etc.).
Good luck, Matt
