Is there any difference between the 99 and new 400exs?

Yep, the new 400exs, 04 and up, have 38 mm carbs, thats about it performance wise.

The 03s and under, im pretty sure have a 36.5 mm carb, not a big difference, just a little better than the rest.

The stock carb on the early model EX's is 35.5mm. I heard in the EXriders forum that the '04 carb size was a missprint - But I dont know for sure.
