You do not have to change shifter, but with these nerfs,.. your brake pedal & spring for it must mount on the "peg" now.(I beleive its the same for 400)
I'm currently modifying the 400ex healguard to fit the"R" w/pro pegs, since AC does not make healguards to fit the "R".AC should make healguards for the "R".

Hey, something I ran into when I changed to the AC Pro Pegs on my R: even though I have size 11 feet, I could only touch the shifter with my boot's toe. The Pro Pegs move the foot placement back a little over an inch, so the shifter is farther away. I fixed my reach problem by going to a Pro Design 1" shorter aluminum shifter (from Nacs, James Lucky, etc), it looks better than the stock, and now it fits my foot the same way it did when I had the stock pegs on.

It makes the Ergos better for racing. It positions you so that assuming a "racing" stance is much easier. Weight Bias is also changed,i guess that could be considered frame geometry.
