Nuts that rattle loose - LT500

I had the crankshaft bearings replaced on my LT500, and after getting it to the sand, and riding for about 1.5 hours, the motor seized.

I brought it back to the mechanic this evening, he popped the side case off, and we discovered the nut that holds the sprocket to the crankshaft had fallen off (weird too since I thought it was back-threaded) and became wedged. Another nut holding the sprocket behind (and slightly higher than) the clutch basket was VERY loose. He reassembled, using locktight (a stronger version of locktight this time), and he assures me that this shouldn't happen again.

Have any of you experienced this. A buddy tells me that he had this happen to his LT250 a while back.
