I'm not sure the reason why manufacturers don't put twist throttles on... but I can tell you why I don't like them. Basically, if the front end of your quad comes up quickly, the riding has sort of a natural reaction to lean forward. Unless you know what you're doing, you can very easily give the quad more gas when you move forward, which is not what you want to do in this situation. That's why I don't care for them. I'm sure the manufacturers have a similiar reason.

There thumbs will get stronger with time. I know mine did because I wanted it to get stronger for those long rides and those long days of fun. I think IceMan11 said it better than I could about the twist throttle. I won't be changing any of mine and I wouldn't dream of putting a thumb throttle on my KDX 200. Chet

I've swapped mine and both of my daughters quads for twist throttles and would never go back to a thumb unit. Americans invented the twist, Jap's invented the thumb unit starting with the Honda ATC 70 and 90. The thumb unit will give faster response because there is a short range of thumb motion from idle to full throttle where as with the twist throttle the range of motion is much greater and thus the control of engine speed is slower but much more controlled. I wanted the control over the speed.

i like a twist throttle on 2 strokes. i had one on my LT and i'd never change that. as long as a quad is not ultra easy to wheelie i'd rather have a twist. if a quad is super easy to wheelie (like my 400) i like to have the thumb just because when u start gettin far back in the wheelie its sooo much easier to control yourself with the thumb. i've had my lt on the grab bar quite a few times and it gets to be a handful but nothin too bad when you get it that far. i say get the twist.

I see it as a problem with steering. When you are on a dirt bike most of the steering you do is by leaning. You rarely ever make radical movements with the handlebars if you want to stay on the thing. On the othe hand to steer a quad you have to move the bars considerably more to steer it especially when the ground is uneven from side to side. If your kids thumbs don't toughen up you can get one of those add on extenders for the thumb nib so they have more leverage.
I had twist on my Banshee and went back to thumb.
