It can be all sorts of things, the Honda gearchange mechanism is not the strongest. The most common problem is the lever or pawl which turns the star wheel on the end of the selector drum. These crack or a bit breaks off. Another common fault is the star wheel loose on the drum, the little bolt comes undone. I always Loctite them in during a rebuild. Both these faults can be corrected by removing the clutch cover and clutches, so no need for engine removal.

Pure coincidence but I had a 450ES in today that would only go into reverse neutral and 1st. It wouldn't go into second with the emergency lever so I thought it would be the mechanism. Sure enough it was the star wheel bolt had come loose. I tried tightening it but it felt like it was going to break so I fitted a new bolt with Loctite. Tried changing by hand and all seems to work. I haven't had time to re-fit the shift motor and plastics yet.
