If it is going to be the old style(00-03) you will also need a caliper bracket from the old style, lower shock shims, lower shock bolt.
I just ordered all the stuff to do the same from 222.

You'll probably need new bushings for the front of the swingarm. They are very hard to get out without trashing them.

Don't even try to re-use them.

Pivot Works should have the kits. I would think Rocky Mt. ATV would have it.

You may need a new brake line too.

Since you ar epulling the axel you may as well replace the bearing carrier bearings.

Can I reuse the bearings or should they be replaced? How will the shock react to the +4 on the swingarm?

Removing the bearings without damaging them would be very difficult. New ones are highly recomended.
