In wyoming if you ride on any public lands, you are supposed to register your quad, which also means you have to buy a license plate.
It is not technically street legal, but I have seen a cannondale driving down the street.
To actually be street legal, you have to have rear view mirrors, a horn and turn signals, I believe.
I thought about it, but decided not to do it.

I think you need mirrors, head light High/low, street legal tires, I dont think you need blinkers but I would any ways (use your hands), and you would have to get your quad licensed for road use. (like getting your car license)
I have also wanted to do this but I dont have enough money or time to do it.

I'm from Germany and we have legalised quads on our normal streets, that means the minimum requirement for the authorities is:
1. To have blinkers.
2. Break lights
3. Covers on your wheels
4. Emission test
5. The noise level is controlled (that means no gun, or trinity or alba racing pipe is aloud.
6. The reverse is only in Swiss a must!!! Extra cost for a DS to implement this revers is about 1700 US$.
