Default rear brakes problem/sprocket placement

on my JM200, the rear brakes make a really loud noise when i jam on them. it still rolls and everything in nuetral but im wondereing if this is a problem. also, the reason my chain always falls off if cause that sprokets dont line up. the back sproket is placed slightly to the right. is this the same on anyone elses quad?

Default rear brakes problem/sprocket placement

sounds like the rear axle is off center, or it is loose. Having problems with the brakes and the chain are leading me to believe this. Lift up the rear and see if the axle carrier has play in it, it's possible the bearing are out there or maybe it's just loose..
