durablue axle problem?

I rode my 250r saturday and then again today saturday the chain cam off 2 times and both times the chained seem tight.Well today it happened again and the cahin was still tight.I adjusted the chain after each incident.I investegated and found my sprocket was lose not my front my rear but it's the spider for the sprocket.I checked to see if my carrier was lose it wasn't is this a prob in the axle or what?It just wiggles from side to side a little bit.It is an eliminator 2+2 when I bought it the spider was on it so is it also covered under the warranty?

humm.. I noticed that the brake carrier moves a little too...but my chain never comes off and there is a little play in mine also not side to side but front to back and that has to be less than a milimeter
