Might just be that the nut has worked loose......
One symptom of bad bearings is that the chain will not stay in adjustment.
If it is the bearings, don't ride it until you sort it out. I rode mine with bad bearings and it ruined the carrier and the axle as well. Got pretty expensive!!

this is very true.....my axle nut comes loss alot even with a rad locknut.
i also have a burgard doube row carrier sitting in my closet when the bearings go bad again.
