driven clutch bolt troubles

I took my driven clutch off to inspect the spring and the clutch buttons. I put it back on and torqued it to the proper 40ft lbs. Well, I didn't wind it correctly so I had to take it off again and re-wind the clutch. Now, I discovered I had a problem. When I was torqung the bolt, it wouldn't get to the proper 40 ft lbs. OI would estimate it at around 30 ft lbs. It actually pulled the washer in and enlarged the whole on the washer to almost the size of the head of the bolt. It also bend the washer. So, I bought a new washer, lock washer, and bolt. But, it did the same thing!!! I don't know what to do. I rode it and nothing went wrong. It ran fine, but I just don't feel to confortable with the bolt not being at proper torque. Any ideas??? Please don't tell me my trans input shaft it stripped.

You Must be careful of what is in the shop service manual because it states "inch pounds" and 'Foot Lbs'....so if it said for example 40 inch pounds..instead of 40 FOOT Lbs....12inch in a foot you would be snaping bolts off cause of over torquing...INCH POUNDS>>>FOOT POUNDS ...big diffrence
