Trail Boss - Chain keeps coming loose, axle clamp turning

(Well I thought I would repost this cry for help, no one responded
to my earlier thread over the weekend, I guess original thread lost in La La land?)
Real quick: Went riding w/buddy at an ORV state park over the weekend. He
had numerous break downs. All relating to his chain spinning loose from his
chain drive shaft. Sitting on the bar beneath practically. On the left side
of the chain, the clamp around the axle would spin/turn. He didn't think
this was normal. Tighten chain with Polaris tool, and the clamp with
Allen wrench. Drive about 200 feet and same event happening all over again.
Loud, clunking noises it was making (I think more from the chain loosening,
hiting frame bar, is there any chain guards to protect this in near future?)
Please help, what could be causing this problem? This is a year
old (2005) Trail Boss, he doesn't think he hit anything (Jumps, rocks
or whatever.)

Maybe a different chain is in order, any suggestions?
What kind of Axle type repair would you suspect
my friend would need? I do know that his TB quad
is out of warranty. (Parts place too, would be
appreciated, much obliged!)

Its a 2005 and maybe I am not describing the clamp
thing right for a lack of a better term. If you are
facing the back of the quad, looking at the seat
and rack, look down and on the left side you have
this enclosed contraption around the axle on the
left side near the middle. That piece (what I defined
as the clamp) is loose and it would spin around. In
front there is a bolt that you can tighten BUT after riding
a little ways, it all comes loose again. Maybe stripped? Don't
know.
To the right (rear facing again) you have the chain on the
sprockets. It is soo loose almost laying on the bar beneath.
We tightened the chain (tool in this hole on disc), pulling tension
and then cranking down.
Soo, it appears we tighten the areas on the right and left side, but
we don't know what is causing this whole problem? Any suggestions
would be appreciated. A little hard to describe in thread, hope I did
a better job!
