Default Wallered swing arm bolt

I just picked up a 95 trail boss 250 on a trade for an axle I had which equals out to CHEAP $$$ wise. Well the SA bearings need replaced and I noticed some play in the bushings at the pivot. Upon inspection, The worst I feared was true. The bold had stripped the nut making it loose and the bolt wallered out the frame hole behind the caliper. Now the question is, How do I fix the frame hole? Also does a bushing kit include the part that goes in the tranny case to support the other end of the bolt? Im gonna assume its wallowed too. I was thinking of grinding the raised part on the frame out (damaged) and welding a grade 8 washer as a replacement. Any better Ideas?
Thanks.

Hey Ragged,,, would you mind posting how you fixed it? I've got a 95 with a bad bushing on the chain side. I'm afraid that it may be in the same condition as yours.

I really didnt fix it. its funny but the bolt came out of the tranny bushing and was like this /in the frame hole. When I cleaned out the bushing the bolt goes straight in and doesnt move anyway. The previous owner didnt use a nylock. And the nut backed off on both sides of the swing arm. I was thinking of using a washer thats the same size as the bushing washer (hole size) in grade 5 or 8, Notch it for the lock tabs, grinding down the bung on the frame the same width as the washer and welding it on. I believe the bolt is 9/16". My bolts were pretty worn from being loose too.
