chain problems??????????????

the chain on my 250x keeps coming loose i have replaced sprockets chains the bolts that hold it tight and nothing seems to kepp it tight doesn anyone know what could be happening to it, what is wrong with it...?

This is just a guess but could the axle carrier be turning inside of the part that clamps tight on it(i can't think of what it's called) but the swing arm .??????
