It sounds like the housing has been damage and no longer has a flat clamping surface so the whole carrier slides in the housing. First remove the housing to check for chips missing or broken pieces. If there are some replace the bearing carrier with a new one. If everything looks fine then wipe off all the surfaces and reinstall it and tighten down the bolts and see what happens. I had the same problem with my 91 and decided to spend the 800 bucks on an aftermarket cam style swingarm. An other thing you might look into doing is to find a use 85-86 lt250r swingarm and replace the one you have with it. The 85-86 has the cam style that the aftermarket companiies use. Make sure your quad has the linkage on top of the swingarm but going this route. The 91-92 models move it belong the swingarm so you can't use the 85-86 style do to different linkage.

Well, first off if you don't have a service manual (cylmer) it might be a good idea to pick one up for you quad. The manaul has detailed instructions on how to do just about everything for your quad, plus the pictures come in handy.
As for what you need to do to remove the housing. First loosen the axle nuts on the sprocket side that hold the sprocket on then remove the wheel/rim on that side. After than remove the hub. Once the hub is off remove the axle nuts completely by sliding them off the axle. Then chain and brake caliper need to be removed next. After the chain is off the sprocket slide it and it's hub off the axle. Now you should be able to pull the axle out of the bearing housing. After the axle has been removed you can remove the bearing housing adjustment bolts and the housing will then be loose so it can be removed. Buy a new housing and bearing and seal kit and install using the reverse order of how you took it apart. For the best price on the stock houosing go and get yourself a dirtwheels or other mag and look thru it and call the different places for price checks on the parts.
Goodluck and if you run across any problems post them on here and someone should answer them.
