Is this a normal one way failure?

Are you sure you didnt put the bearing back on backwards docdave? Its hard to tell, the little ridge goes in toward the flywheel

I bet your stock gear is fine also since it went all at once when the final bolt broke and didnt get a chance to gaul up like one thats been used slipping for a couple months.

OEM =$166
L&A = $189

If you factor in getting the gear its exactly the same price as yamaha retail for something better with a warranty. Some places the gear is $54 instead of near $80

I put mine in just a little under a year ago and its still going strong. Then when they were released I put one in a bike with a used and gauled up gear (not too bad) and neither have had problems so far. With the amount of L&A one-ways in raptors now and the amount L&A has gotten back defective(easily counted with the fingers on one hand) so far they are nearly indestructable in my book.

I may have put it on backwards. Is there a recess for the bolt heads on both sides?

nope your right, didnt think of that. they are'nt recessed both ways so it wouldnt go together wrong

If you took all the sprags out and put them in backwards then it could free wheel backwards. If it free wheels both direction then you have some real bad wrong.

I didn't take the sprags out. The bearing was still in one piece when I removed it.
