you could probably do a search under rancher winch mount and find the part number. i ordered locally from cycle country for about $66 for the mount and fairlead. why spend $66? it is just a flat plate that attaches with c-clamps, i made mine and have had no problems and it cost just the price of the clamps.

Good man, Rancher; do it yourself and save! But, "c-clamps?" Not "U-bolts?"
My certified welder cuts plate with a torch freehand with edges as smooth as if bandsawn with a pattern jig. He can certainly make a serviceable custom winch mount plate.
Also, while there's nothing wrong with roller fairleads, hawse fairleads work just fine.
