The dogs on the second gear are most likely rounded off and will slip out of the gear they mate to. Mine does it occasionally at the dunes when under a high stress situation like climbing a steep hill. If yours slips out rather easily, you should replace second and third at the least and do the case inserts while your at it if you haven't already.

I just got done dealing with that problem,on a 90.It's hard to say anything impiticular with out cracking her open,but there is a very good chance it's a bent or worn fork, or bent fork shaft, or a fork to gear clearance issue.I just bought brand new fork's and they came on the low side of the tolerance almost out of spec (I LOVE HONDA). but it could also be a that the shifting drum,pawl's,pawl springs,or cam stopper(shift drum stopper)spring,is worn. i was building a new engine so i replaced it all and it worked but in most cases i've seen everything i talked about will show some wear, so unless some thing sticks right out as being the cause, it can be tricky to diagnose.And if you try to cut corners theres a good chance you'll be craking her open again (NOT FUN).Tranny's can be tricky...........

This sounds a little stupid but my 87 was doing the exact same thing with a rebuilt engine and tranny , and all that was wrong was I somehow managed overfill my crankcase by 100 - 150 ml of oil and it made my tranny do all kinds of neat things like not shifting out of some of the gears , and popping out of other gears , I was sh!ttin myself for a while thinking I screwed up putting it together and I was going to do it all over again , drained out the excess oil and I was good to go.
