How long have you been riding for???
When I first started my thumb killed, but afterwards you start working those muscles and the paint goes away. I don't feel anything now. I also took my throttle housing, and turned it up more so I wouldn't be pushing with the tip of my thumb but more so with the whole finger.
You just have to get used to it.

The Cascade throttle extender is very comfy and eliminated any sore thumb problems. It sticks out further from the bars for more leverage. This also makes it easier to use the palm of your hand which is nice in cold weather or when traveliing longer distances at a constant rate of speed. The only downfall is if you ever go over the bars, the lever is very likely to snag on your pants as you 'fly' by. Still, I feel it was $40.00 very well spent. (I have seen other brands cheaper{Moose?}, but don't remember where off hand.)
