This does happen often. The problem is after the hub has been removed, it is sometimes hard to get the axle nut tight and still get the cotter pin to lign up with a hole. Usually to get them lined up and be tight, you have to torque them above factory spec.
When you put the new hub on, grease the splines of your axle. This will keep the hub and axle from grinding on one another if the hub comes loose again. It will not cause the hub to slip on the axle unless your axle splines are stripped as well.
